Project Description
People living with HIV and noncommunicable diseases (NCDs) have a high mortality post-discharge in sub-Saharan Africa. This is in part due to dis-integrated linkages of care post-admission for HIV/NCDs. This project will address the HIV/NCD care cascade after hospitalization to reduce adverse outcomes post-discharge.
